ABOUT OUR PRESENTERS

Colleen Hagan
Director of Client Care & Training
Colleen leverages over a decade of home and dementia care experience to enhance Full Bloom's person-centered approach. She manages complex care plans and caregiver coordination, develops training programs, and supports families, ensuring compassionate care for those with dementia.
Colleen is a Teepa Snow-certified dementia care professional, and she holds multiple certifications through the Positive Approach to Care® organization, and is a certified Alzheimer's Association Community Educator®. She skillfully combines these certifications with evidence-based strategies to design care plans that empower both caregivers and families. Her prior work as an electron microscopist at Massachusetts General Hospital enriches her perspective, blending scientific insights with a deep understanding of the human aspects of dementia care. Colleen is recognized for her warmth and ability to build trust, leading care teams and collaborating with families to provide high-quality in-home care.

Tracy Hoeft, RN
Partnership Development Manager
Tracy Hoeft has over 30 years of nursing experience and excels in sales and relationship management as the Partnership Development Manager at Full Bloom Memory Care. Known for her compassion and connection-building skills, Tracy is a certified Community Educator through the Alzheimer's Association® and a trusted leader in senior services. Her background as a Registered Nurse has equipped her to handle complex health and personal challenges effectively, ensuring optimal outcomes for her clients. Her dedication to building trust and mutual respect is evident in her sales and relationship development roles.
Inspired by her father’s experience with Alzheimer’s, Tracy launched the Vintage Moves program in 2023, using music and dance to engage seniors with memory changes. Her career highlights her skill in integrating clinical knowledge with strategic sales approaches, having held impactful positions at TheKey, National General Insurance, and Ascension.
